For physicians called to move beyond traditional healthcare settings and bring dignified, relationship-centered care directly to people living on the margins.
The Street Medicine Fellowship is a transformative year of clinical, community, and mission-driven training designed for physicians committed to serving individuals experiencing homelessness and extreme vulnerability. Fellows learn to provide whole-person care where patients actually live… under bridges, in encampments, inside shelters, and throughout the overlooked spaces of the city.
This is not medicine delivered from a distance.
It is medicine practiced through presence, trust, consistency, and deep respect for human dignity.
As part of the Cahaba Medical Care network, fellows are immersed in a model of care shaped by justice, compassion, and a belief that healthcare should move toward people… especially those too often left outside traditional systems.

Street Medicine asks physicians to practice differently.

It requires flexibility, humility, creativity, and the ability to build trust in environments where healthcare systems have often failed the people they were designed to serve. Fellows learn to provide high-quality care in low-resource, nontraditional settings while working alongside shelters, outreach teams, behavioral health professionals, and community organizations.
Here, medicine becomes deeply relational.
You learn to move toward people others often pass by… building continuity, dignity, and trust in places where healthcare is rarely designed to go.
THE FELLOWSHIP EXPERIENCE
Street Medicine Rounds
Participate in collaborative outreach alongside multidisciplinary teams and community partners to provide direct medical care in the field.
Shelter-Based Care
Provide both acute triage and longitudinal medical care within local shelters and shelter-based medical units.
Addiction Medicine
Develop expertise in low-barrier treatment for substance use disorders, harm reduction, withdrawal management, and recovery support.
HIV & Infectious Disease Care
Gain experience managing HIV, anti-retroviral therapies, infectious disease prevention, and complex chronic illness in underserved populations.
Advanced Wound Care
Learn specialized treatment of chronic wounds, soft tissue infections, burns, and injuries commonly encountered in unsheltered environments.
Advocacy & Systems-Based Care
Work at the intersection of medicine, housing instability, behavioral health, and public systems while learning to advocate for patients navigating fragmented care structures.

Complete away rotations with nationally recognized Street Medicine programs to expand clinical experience and strengthen collaborative networks.
